Complete the application form below by indicating your Membership Type and providing your Membership Information. Providing an accurate e-mail address is important because it enables us to provide you with with the latest announcements regarding events and other FMS news. When you have completed all of the fields, please select the Continue button.

 
Step 2. PRINT the Membership Application Confirmation page that contains your Membership information.

  
Step 3. MAIL the printed Confirmation page along with payment of your Membership fee based on the Membership type you have chosen. You will be sent an acknowledgement of your Membership welcoming you as an FMS Member!
